    Students in the Nurse Refresher Course must provide proof of the following documentation TWO WEEKS PRIOR to the start date of class. (The documentation is not required at time of course registration.)

    GA Board Application Verification Form

    STUDENTS WHO DO NOT HAVE AN ACTIVE RN LICENSE IN GEORGIA

    Students from another state (ENDORSEMENT) or those lapsed in Georgia (REINSTATEMENT) must fill out an application with the GA Board of Nursing, prior to registering for our course. Once application has been made to the GA Board, sign the GA Board Application Verification Form for our file. Kennesaw State University will send GA Board the Form A (temporary license request). GA Board will mail a Temporary License to your home address near the end of the theory portion of the course, before clinical begins.

    GA Board Jurisprudence Exam (Nurse Practice Act)

    STUDENTS WHO DO NOT HAVE AN ACTIVE RN LICENSE IN GEORGIA

    Students are required to complete and RETURN the Jurisprudence – Georgia RN (Nurse Practice Act) Certificate of Completion. Your certificate must be in our office before course begins. Information about the Georgia Registered Nurse Jurisprudence Exam can be found at the NCSBN Learning Extension. It is recommended that you review the Nurse Practice Act, Board Rules and the Scope of Practice Decision Tree, found on the GA Board of Nursing website menu under “Laws, Rules and Policies” before starting the exam.

    Criminal Background Check

    Criminal Background Check/Drug Screening (required for clinical access) must be conducted by a vendor designated by the WellStar College RN Refresher Program. Dismissal from the program may result if you are not capable of meeting clinical agency requirements for criminal background check and/or are found to have a positive drug screen. Those with background issues need to contact us prior to registration for advisement on obtaining required temporary license. Previous criminal conviction may prevent admission into the course.

    CPR

    REQUIRED through the American Heart Association, BLS for Healthcare Providers (no exceptions).

    AMERICAN HEART ASSOCIATION CLASS SCHEDULES

    Turn in copy of CPR card, front and back, with your signature. Must be current through the end of clinical rotation.

    Malpractice Liability Insurance

    Submit an official MEMORANDUM or CERTIFICATE (on‐line confirmation not acceptable).

    Limits of Liability must be at least $1,000,000/$4,000,000.

    Although health insurance is required, if the student’s circumstances do not permit them to have health insurance, KSU will allow the student to sign a waiver of liability.

    International Nurses

    International nurses educated outside the U.S. must submit acceptable TOEFL (Test of English as a Foreign Language) scores: 79 IBT (internet based), 540 PBT (paper based), or 207 CBT (computer based)
